Job Description:
A company is looking for a Family Clinical Advocate, Registered
Nurse - Remote.
Key Responsibilities:
Provide telephonic and digital case management to improve members'
health and well-being
Develop and implement care plans in collaboration with healthcare
providers and members
Coordinate patient education and community resources to support
health goals
Required Qualifications, Training, and Education:
Current multi-state/compact Registered Nurse licensure in state of
residence
Microbiology course noted on nursing school transcript or college
pre-requisites
Minimum of 3-5 years clinical experience; telephonic case
management experience preferred
Specialty experience in areas such as NICU, Oncology, Transplant,
Maternity, or Behavioral Health preferred
Training in motivational interviewing preferred
